Many patients who have undergone elective sterilization also known as tubal ligation surgery wonder which treatment option will benefit them best. If you have had a tubal ligation, you have two options to conceive again and increase your chances of pregnancy; tubal ligation reversal surgery or In-Vitro Fertilization (IVF). Each case is different and what is best for one patient may not be the best option for a different patient.

The best option for you is going to depend on your general health, your age, your partner’s sperm how much damage your fallopian tubes have as the result of your initial tubal ligation surgery. It is important to choose a qualified physician with experience performing successful tubal ligation surgeries. The first step in getting started is making a consultation appointment with Dr. Shaykh. At this appointment he will review all of your medical history, and perform an exam/ ultrasound to ensure there are no underlying problems with your uterus or ovaries. At your consultation appointment with Dr. Shaykh, he will also extensively review your surgical report from your tubal ligation to see how much fallopian tube is stated to have been left remaining by your doctor after the surgery was completed. Based on these findings as well as your age, Dr Shaykh can give you his best recommendation for treatment options; either tubal reversal or IVF.

If you want to determine if you are even a candidate for tubal ligation reversal surgery, you may send our tubal reversal center a faxed copy of your operative report from the tubal ligation surgery and include your phone number and name on the cover sheet. Dr. Shaykh will review your surgical report and determine if you are a good candidate for tubal ligation reversal. The findings within your surgical report, is going to be a big deciding factor in which treatment option Dr. Shaykh recommends. There are many different types of tubal ligation methods and the success of reversal varies based on the type. Typically, if you are young and your surgical report states that the majority of your fallopian tube remains, Dr. Shaykh will recommend tubal ligation reversal surgery. This usually produces the highest success rate for the lowest cost, pending your surgical report is correct. However, if your surgical report states that the entire length of your fallopian tube was removed, severely damaged or if you are over a certain age, he may recommend IVF instead if tubal reversal to give you the best chance at pregnancy.

Another very important point to consider is the quality of the sperm from the patient’s current partner. It is best if both partners come to the consultation appointment so that Dr. Shaykh can review the reproductive history of the male. He will be asked if he has fathered any children before. If yes, typically there is not a reason to believe there is a problem with the sperm and tubal reversal surgery s likely to be recommended. If no, however Dr. Shaykh may recommend a semen analysis to be performed at the time of the consultation to determine that the man does not have a problem with his sperm. If there is a problem with the sperm, IVF treatment with intracytoplasmic sperm injection (ICSI) may be recommended as giving him a better chance of achieving pregnancy versus of tubal reversal surgery. If there does not appear to be a problem with the sperm but the patient still desires to have a semen analysis performed, Dr. Shaykh would be happy to perform this test upon request.

In addition, the actual amount of scar tissue present on the fallopian tubes and the condition of the tubes at the time of surgery is what will determine the most accurate chances of success following tubal reversal surgery. Before surgery, Dr Shaykh will give you his best recommendation for what he thinks your chances of pregnancy will be after reversal as well as what your probable chances of conception would be with IVF and let you make the decision on how you would like to proceed.

In some cases, where the amount of tube remaining is questionable or unclear from the surgical report and if the patient desires, Dr. Shaykh can perform a diagnostic laproscopy at the time of the scheduled tubal ligation reversal surgery. A laproscopy is basically where a small incision is made and a scope is inserted through the belly button so the doctor is able to visualize the tubes to see if they can be repaired with traditional tubal reversal surgery. If they can, he will remove the scope and go ahead and perform the reversal at this time. If some reason, they cannot be repaired he will not perform the surgery and the patient will only be charged for a diagnostic laproscopy and will be credited back any additional funds that they paid in anticipation for tubal reversal surgery.

As far as pricing goes, tubal reversal surgery is going to be lower in cost than in-vitro fertilization. The cost is an important factor to consider as well because many insurance companies will not cover anything related to IVF treatment if the patient has had an elective sterilization (tubal ligation). In contrast, with in-Vitro fertilization, procedure cost is slightly higher than tubal reversal. This makes tubal reversal a lower cost option instead of IVF if the patient is a good candidate for reversal.
